Equipment Pick Up dates and times are on the books!
June 25th: 6-8PM - 8th grade only
June 26th: 6-8 PM - 7th grade (8th grade welcome)
June 27th: 6-8PM - 6th grade (7th and 8th grade welcome)
Players must attend on their assigned day or a later one. We are unable to accommodate players on an earlier date even if they have an older sibling.
The fittings will be done on the lower level of CHS. (go through the main entrance and proceed to the lower level)
All the paperwork listed below and deposit check are REQUIRED for the release of equipment. Be sure to read the information below carefully to ensure you bring everything necessary for your player to receive his equipment. Pay particular attention the date of the MD physical/clearance form as it must comply with GMSAA rules.
A deposit check of $500.00, written out to CAMBRIDGEFEEDERFOOTBALL: Please put your player's name and grade on the MEMO part of check. Checks will be held until equipment is returned at the end of the season.
Birth Certificate: Copy only. No originals please.
Sports Physical Form: *This form will need to be completed by you and then signed by your player’s physician. At this age, you may have already completed this form as a part of your child’s annual well visit with his pediatrician. If so, call your child’s pediatrician office to request a copy of the signed form. *If a Sports Physical Form has never been completed for your child, you will need to print the form off the website, complete and bring it to your child’s doctor for signature. IMPORTANT:IT MUST BE DATED AFTER NOVEMBER 17, 2017 to be current during football season. If your son's physical is dated earlier than this you will need to get another one done. This is a GMSAAA rule. Most urgent cares will be them for $25.
GMSAA Residency Certification Form: This form certifies that your player resides in the Cambridge High School District.
ImPact Consent form: For NEW players only- Signed by player and parent.
GMSAA Concussion Awareness Form: This form confirms player(s) and parent(s) have read and understand the GMSAA concussion information and policy. Parent and player must sign this form.
Player & Parent Commitment and Code of Conduct Agreements: Your player should read and sign the Player Application Form. Your player will need to know what is expected as far as his effort, code and conduct on the field. Parents should read and sign the Family Guideline form which outlines the rules and guidelines you will need to follow.
GMSAA Release and Waiver of liability, Assumption of Risk and Parental Consent Agreement Form: Waiver, risk and parental consent agreement is required by GMSAA.
